The word works both as an identity (“I’m a sadomasochist”) and as a description of a scene (“that was an S&M scene”, meaning one built around sensation such as impact, wax, or needles). S&M is one of BDSM’s three axes, alongside B&D and D/s, and the sadomasochist is the person comfortable at both ends of it.
